Due to the expansion of our service, an exciting opportunity has arisen to join our dedicated Care at Home and Enablement Team within the East Ross area. This role is ideal for individuals who are passionate about supporting people in their own homes.
Key Objectives of the Enablement Worker Role:
Maintain, maximise, and promote independence for vulnerable individuals, enabling them to remain safely in their own homes.
Prevent hospital admissions through proactive support and early intervention.
Facilitate effective hospital discharges by supporting individuals in their own home.
Support ongoing assessment of clients in their own homes.
Contribute to person-centred, goal-focused care plans, ensuring that support is tailored to individual needs and aspirations.
Provide ongoing review and support to aid the transition back to independence, reducing reliance on long-term care services.
This proposal aligns with strategic priorities around early intervention, reablement, and reducing delayed discharges, while also improving outcomes for service users.
Applicants should be able to demonstrate a self-motivated and dynamic approach. They should have good team working abilities. Have the ability to participate and communicate effectively. Work as part of a larger, multi-disciplinary team whose goal is to deliver person centred care whilst ensuring health and social care standards are maintained along with NHS values. Work will be overseen by a senior enablement worker and the Care at Home Officer.
Applicants should have or be willing to work towards an SVQ 2.
Previous experience is desirable but not essential.
A current driving license, as well as use of your own car is essential for this role.
