Hospital Social Worker (Part Time) Salary - £22,037 – £24,641 actual salary (Grade 7) Hours - 18.5 Contract type - full time, Permanent Work Style - Field: working in communities across the city Join us at this exciting time as a Hospital Social Worker within our citywide Hospital Social Work team. You’ll play a key role in supporting timely discharges from all hospital settlings We’re looking for passionate, person‑centred practitioners committed to strengths-based social work and excellent practice. At our award‑winning council, we use the Three Conversations© approach—building strong, restorative relationships and focusing on what matters to people. About the Role Support people leaving hospital or Discharge to assess services, working closely with carers and support networks. Lead and influence safe, timely discharges as part of a multidisciplinary team. Understand funding routes and relevant legislation (Care Act, MCA, discharge policy). Produce clear reports, assessments and plans, and monitor positive outcomes. Champion strengths-based practice and ensure individuals’ voices are central. Work collaboratively with partners including health, police, voluntary sector and providers. About You Be a registered Social Worker (SWE). Have completed your ASYE or equivalent. Hospital experience will be desirable. Have strong analytical, assessment, organisational and communication skills. Be committed to professional development and high-quality practice.
