Clinical Pharmacist - Independent Prescriber (IP)
This role supports general practice staff with prescription and medicines management across multiple sites. The post holder will manage repeat prescriptions, handle acute prescription requests, perform medicines reconciliation during transfers of care, and provide clinical medicines advice to improve patient safety and outcomes.
Responsibilities
* Reduce GP time spent on medication reviews by conducting clinical medication reviews in appointment slots.
* Review ongoing need for medicines, monitoring requirements and support patient medication use.
* Manage minor ailments triage and provide patient‑facing clinics for medication queries.
* Reconcile medicines following hospital discharge, intermediate care and care homes to ensure continuity of supply.
* Perform targeted medication reviews in high‑risk patients (e.g., frail elderly, poly‑pharmacy).
* Contribute to multidisciplinary medication‑related reviews and public health campaigns.
* Audit prescribing, implement drug withdrawals and alerts, and improve repeat prescribing processes.
* Provide immunisations (influenza, travel) and support management of drug dependence and alcohol‑related disorders.
* Signpost patients to appropriate services to enhance general health and well‑being.
* Maintain accurate and updated prescribing documentation and report to senior staff.
Key Qualifications
* Current UK professional registration as a pharmacist.
* Experience working within a Patient‑Centred Care Network (PCN) or similar primary care setting.
* Experience with EMIS or equivalent GP systems.
* Evidence of working within a multidisciplinary team (MDT).
* Commitment to completing an independent prescribing course within six months of the probationary period.
