Overview
An exciting opportunity has arisen for a Community Practitioner to join our new Intensive Support Team (IST) working alongside the Community Autism and Learning Disability Service.
This is a community-based, specialist team preventing crisis, reducing hospital admissions, and supporting people in the least restrictive setting. The service will be integrated at a local level and provide psychological and psychiatric person-centred interventions. The team is intrinsically linked with the dynamic support register and offers support over the 7-day period and aims to proactively target people with learning disabilities and/or autism who are most at risk of unplanned admissions due to mental illness or behavioural presentation.
Responsibilities
* Monitoring the performance and quality of the service the team delivers
* Clinical leadership and effective day-to-day management of the team
* Commitment to promoting recovery and maximising independence, fulfilling wider management responsibilities as required by the organisation and performance management
* Provide day-to-day management of the team, maintain excellent communication and build positive working relationships with Systems Partners
* Support with recruitment, retention, and induction of staff
* Conducting clinical and managerial supervision, appraisals, and oversight of individuals' development
* Managing performance and absence/staff wellbeing
* Ensure statutory and mandatory training is completed
* Support implementation and development of patient and carer experience
* Oversight of informal and standard complaints process
* Engagement within interface meetings
* Address grievance and disciplinary cases
* Motivate, engage, and develop the team within a multidisciplinary environment
* Lead and coordinate team communication, including holding regular team meetings, communicating and consulting on service changes, and acting as a conduit of information
* Support the team following serious incidents and with debriefs, local and after-action review processes and serious incident panels
* Support staff offering learning and training opportunities, clinical supervision, and a supportive, collaborative team culture
Qualifications
Experience and knowledge of supporting individuals with a learning disability and autism.
Ability to influence service development and play a key part in shaping how we deliver community care to people with learning disabilities and/or autism.
