Responsibilities
* Deliver specialist neuro‑rehabilitation through advanced assessment, clinical reasoning, and evidence‑based intervention for adults with a wide range of neurological conditions.
* Lead complex case management, coordinating personalised rehab plans that support people to maximise independence in their homes and communities.
* Provide clinical leadership within the multidisciplinary team, offering expert guidance, joint working, and specialist input into complex decision‑making.
* Supervise and support junior staff, students, and therapy assistants, helping to build skills, confidence, and high‑quality practice across the service.
* Drive service development, contributing to quality improvement projects, pathway redesign, and innovation in community neuro‑rehab delivery.
* Build strong partnerships with patients, families, carers, and external agencies to ensure seamless, person‑centred care.
* Maintain high professional standards, including accurate documentation, outcome measurement, and ongoing CPD to keep practice at the cutting edge of neuro‑rehabilitation.
