Responsibilities
* Provide consistently high standards of care for patients and families to ensure the best outcomes.
* Work closely with all members of the team to deliver safe care and achieve role‑dependent duties within the scope of your training and competence.
* Communicate compassionately and effectively with patients, families, carers and healthcare practitioners, maintaining confidentiality.
* Deliver personal care to patients while maintaining their privacy and dignity and support rehabilitation.
* Act within the scope of your training and competence, asking for help when unsure.
* Follow Trust standards, policies and protocols.
* Observe, assess and respond to personal care needs, escalating concerns to nursing staff as required.
* Organise tasks or deliver patient care with supervision from a Senior Health Care Assistant or nursing staff.
* Undertake personal care duties and support activities of daily living (mouth care, hygiene, toileting) with person‑centred care.
* Safely deliver care duties including manoeuvring patient equipment such as hoists and wheelchairs.
* Under supervision of a registered practitioner, undertake delegated procedures within training limits (e.g., moving and handling, personal care).
* Support feeding, diet and nutrition; record intake and output accurately and report changes.
* Ensure patient property is managed according to Trust policy.
* Monitor supplies and order as appropriate.
* Demonstrate duties to new starters and contribute to the learning environment.
* Maintain a safe, healthy working environment.
* Act in the best interest of individuals, following duty‑of‑care principles to prevent harm.
* Work in partnership with individuals, their families and the wider team.
* Maintain and develop your skills through learning activities and appraisal.
* Participate in team handovers.
