We have transformed our clinical pharmacy service across NHS Fife in an ambitious project to join up care to operate as an integrated pharmacy service across hospital, primary care and specialist teams. This ensures elderly care services are joined up Fife wide to care for our patients across transitions of care and ensure the best outcomes for patients with their medicines.
An exciting opportunity has arisen for experienced pharmacy technicians registered with the GPhC who have proven post qualification experience, to join our integrated pharmacy team, working across Fife as part of the community health clinical pharmacy team.
You will be based in the Victoria Hospital, working within the supported discharge wards and the rapid assessment and discharge unit, as well as across Acute Services, Community Hospitals, and Hospital at Home.
You will work closely with your clinical pharmacy colleagues to undertake medicines reconciliation to prioritise the safety of the elderly population. Additionally you will work closely with the multidisciplinary teams to support and coordinate discharge flow and pathways from the Frailty/Care of the elderly wards
You will play a crucial role in providing a safe, effective pharmacy service Fife wide, ensuring smooth transitions of care. You will also support the timely and safe provision of medicines for inpatient care, discharges, and transfers.
The role will involve travelling across Fife to cover other community hospitals out with your base locations. You will also have sessions to enable you to maintain your checking qualification.
You must be able to work independently or as part of the pharmacy team and wider multidisciplinary team. You will be an excellent communicator who has the ability to build good relationships with colleagues and other healthcare professionals. You should have experience of working in a clinical environment and managing staff.
Due to legislative changes from 1 April 2025, this post may require a different level of criminal records check done than is currently the case.
