Looking for a role where you can help people turn their lives around? As our Lead Support Worker (known by us as Support Coordinator), you'll support customers with complex needs in their homes and out in the community.

What you'll do
* Create person-centred support plans and lead a small team to deliver them
* Support customers with mental health, substance use, homelessness and isolation
* Help people build daily living skills and confidence in their community
* Carry out risk assessments, reviews and safeguarding checks
* Set up and end tenancies, making sure customers feel safe and supported

* Experience creating support plans and leading others to deliver them

Practical bits
* You'll work Monday to Sunday on a rota-based system
* You'll work 37.5 hours per week on a rota-based system
* You'll cover a patch across services in West Yorkshire
* You need a vehicle insured for business use, and we pay your mileage
* You need an Enhanced with barring list DBS check that we pay for
