Overview
The post holder will work under the direction of the Children's Community Manager with responsibility for delivering community-based paediatric nursing care and leading the clinical team as required.
Responsibilities
* The assessment of care needs for all patients, ensuring appropriately planned care is implemented and evaluated.
* Work with minimal supervision and be designated to take charge and work as a lone worker within the community on a regular basis.
* Provide care at home without supervision to facilitate the prevention of admission or reduction in hospitalisation.
* Act as an effective clinical leader and ensure the clinical and professional development of all staff for whom they are responsible.
* Assist in providing clinical cover in all areas of the paediatric unit when required.
* Assess individual care needs, develop, implement, and evaluate care in conjunction with the multidisciplinary team.
* Provide efficient, effective and evidence-based nursing care to children and young people within the community setting.
* Perform nursing procedures to a high standard in accordance with the Royal Free Clinical Practice Guidelines and policies, ensuring these policies are understood and adhered to by all staff within own management responsibility.
* Recognise and manage life-threatening emergencies in children and be able to carry these out in the community setting.
* Highlight and identify child protection concerns to the team leader/child protection lead and activate concerns through child protection procedures; ensure the team adheres to child protection policies and guidelines.
* Maintain accurate and comprehensive nursing records using appropriate documentation and the Nursing Information System, with confidentiality and safekeeping in accordance with NMC and trust guidelines.
