We have an exciting opportunity for two Social Care Practitioners to join the Transitions Team at Swindon Borough Council. The successful candidates will join a friendly, supportive and dynamic team. We are committed to providing good quality supervision and continuing professional development opportunities.

There are two position available one for 35 hours per week and another at 37 hours.

Your Next role

You will work closely with young people and their families as well as several professionals' including Education, Health, Children Services and supported employment services amongst others. The Transitions Team support young people from the age of 14 years to 25 years, and you will be responsible for completing the Care Act Assessment in preparation for the young person turning 18 years old and transitioning to Adult Social Care.

You will follow the care management process and complete reviews, support planning which may also include Mental Capacity Assessments, and you will be part of the Transition's duty rota.

We are interested in applicants who have a passion to work with young adults who may have a learning disability, physical disability or mental health needs and are committed to providing a person centred and strengths-based approach to their work.

You will demonstrate good communication skills and enjoy working in a fast-paced environment. The role entails working in partnership with adults and carers to assess need, set up outcome focussed support plans as appropriate as per the requirements of the Care Act 2014.

You will need experience working with young adults with complex needs. You must hold an NVQ Level 3 in Health and Social Care (or equivalent). You must understand relevant legislation and the ability to work independently and manage a varied caseload.

Candidates must be fluent in the English language (as a requirement of Part 7 of the Immigration Act for the effective performance of a customer-facing role).

A valid driving licence and access to a car is essential for this role.
