1 year contract with a Local Authority Job Summary: • Hackney Council is seeking a dedicated and experienced Personal Advisor to join the Leaving Care Service within the Children, Adults and Community Health Directorate. • The post holder will support care leavers aged 18–25, helping them transition successfully into adulthood by ensuring access to suitable accommodation, education, training, and employment. • Working as part of a multi-disciplinary team, the Personal Advisor will build meaningful relationships with young people, coordinate their Pathway Plans, and provide practical and emotional support to promote independence and positive life outcomes. Key Duties/Accountabilities (Sample): • Build trusting, professional relationships with care leavers to assess needs, identify risks, and co-develop Review Pathway Plans. • Maintain regular contact with care leavers, ensuring their voice and aspirations are reflected in all planning and decision-making. • Liaise with education, housing, health, and employment services to coordinate multi-agency support for care leavers. • Provide practical assistance to young people in developing independence skills, including budgeting, tenancy management, and accessing community resources. • Monitor accommodation arrangements to ensure safety and suitability, escalating safeguarding concerns where necessary. • Prepare clear, evidence-based Review Pathway Plans and reports within agreed timescales. • Attend and contribute to multi-agency meetings, Looked After Children Reviews, and Child Protection Conferences where applicable. • Support young parents and care leavers with additional vulnerabilities to access specialist services. • Maintain accurate and up-to-date case records, ensuring data is entered promptly into relevant systems. • Work flexibly to meet young people’s needs, including occasional evenings and weekends. Skills/Experience: • Substantial experience of direct work with young people, care leavers, or vulnerable families. • Strong understanding of child development, adolescent behaviour, and the transition to adulthood. • Experience supporting young people to access education, employment, training, and housing. • Sound knowledge of the Children (Leaving Care) Act 2000, Children Act 1989, and relevant Care Leaver guidance and legislation. • Ability to assess and balance risk and protective factors and act decisively to safeguard young people. • Excellent interpersonal and communication skills to build rapport and motivate care leavers. • Confident in multi-agency working, coordinating services, and sharing information in line with Data Protection and safeguarding protocols. • Strong organisational skills with the ability to prioritise and manage a demanding caseload. • Competent in IT systems, case recording, and producing professional reports. • Commitment to equality, diversity, and inclusion, and to promoting positive outcomes for care leavers. Additional Information: • The closing date: 23/10/2025. • NVQ Level 3 or equivalent qualification in Child and Family Work or a related field (essential). • Location: Hackney Service Centre, London. • Contract: 10/11/2025 – 15/11/2026 (Full-time, 37 hours per week). • Compliance: Enhanced DBS required (Children & Adults). • Working Pattern: Hybrid / office-based as required; flexible working including evenings/weekends.
