An opportunity has arisen for a full-time permanent SocialWorker to join the Changing Futures Sussex Programme within East Sussex CountyCouncil - Adult Social Care - Mental Health Service.
Changing Futures is a Pan Sussex system change Programmefocussing on improving the system for people experiencing multipledisadvantage. It is one of 15 national Programmes bringing together public andcommunity sector partners to redesign ways that local systems and services aredelivered, designed, and commissioned to better meet the needs for adultsexperiencing multiple compound needs (MCN).
Multiple compound needs are defined by the national ChangingFutures Programme as people who experience three or more of: homelessness,current or historical offending, substance misuse, domestic violence, andmental ill health.
Main duties of the job
The Changing Futures Programme Sussex aims to create anenvironment where individuals experiencing multiple compound needs, whorepeatedly fall through the net of service provision, receive flexible, traumainformed, person-centred support. The Programme aims to enable positive changesin services; to make them better connected, and easier to access. ChangingFutures Sussex works in partnership and is embedded across the three LocalAuthority areas: East Sussex, West Sussex, and Brighton and Hove.
The Social Worker will be part of a multi-disciplinary teamwho will adopt a curious, creative and collaborate approach to serviceprovision. We are looking for innovative thinkers and those that truly believeand practice a trauma informed approach. The team will be part of the widerChanging Futures Sussex Systems change priorities to identify blocks andbarriers in the system, recognising best practice and flexible working toovercome the challenges facing people with multiple complex needs.

About us
The role covers service delivery across East Sussex. Hybrid working arrangements are applicable to the role, this includes elements of office-based working, client visits and working from home. The office base for this role is St. Marys House, Eastbourne. You will need to demonstrate you can meet the travel requirements of the role in your application.
What we are looking for:
A registered social worker, experience working directly with vulnerable adults and their families, completing assessments, using legal frameworks, and creating care plans. Knowledge of psychological development, systemic approaches, and interventions.
* Working in a mental health setting with clients experiencing multiple complex needs.
* Post graduate professional qualification (such as CQSW/CSS/DipSW or BA/MA in Social Work or equivalent and PQSW)
* Previous experience working within a multi-disciplinary team, MDT, would be desirable or evidence working in partnership with other agencies to gain better outcomes for clients.
* Knowledge and application of health and social care legislation, safeguarding procedures. MCA and Care Act 2014 is essential.
* Sound and accurate IT skills for general office duties, producing reports and research.
* Registration with Social Work England and responsibility for maintaining registration; significant post-qualification experience in dealing with diverse issues.
Job responsibilities
1. Undertakea range of tasks which could include investigation of adult safeguardingissues, carrying out assessments of need, analysis and planning, assessing andmanaging risk, commissioning support and reviewing.
2. Identifyand appropriately report children and adult safeguarding concerns, liaisingwith key agencies to gather information and acting as Investigating Officer asdelegated.
3. Workwith a range of legal interventions and departmental policies and procedures tosupport outcome-based planning for adults.
4. Workdirectly with adults in need, carers, families and their communities to supportpeople to make informed decisions and choices, empowering them to express needsand participate in the decision-making process.
5. Workwith partner agencies and adults in need and their carers and families toenable them to build networks of community support and maintain theirindependence for as long as possible.
6. Maintaingood case recording and present reports to panels and in court as appropriate.
7. Contributeto the learning of social work students, newly qualified social workers andother team members, for example through group supervisions and mentoring.
8. Evidenceongoing commitment to continuous professional development and undertakerelevant training in line with the Professional Capability Framework.
MentalHealth addendum
9. Inaddition to the tasks outlined above, Social Workers working in Mental HealthServices will also be required to do the following:
10. Totrain as an Approved Mental Health Professional (AMHP) two years aftersuccessful completion of the formal ASYE programme.
11. To actas an AMHP and contribute to AMHP rotas as required. This may involvecontinuing to work after 5pm on duty days.
12. Tocomplete relevant training in order to provide practice placements for AMHPstudents in training.

* Social work qualifications and registered with the professional regulatory body.
* Successfully completed Assessed and Supported Year in employment, or the equivalent if prior to 2012, (NQSW year and academic module) or equivalent.
* Academic /post graduate learning relevant to the post.
* Level 1 Practice Educator Professional Standards (Enabling Learning or equivalent).
* Demonstrate an understanding of social welfare policy.
* Demonstrate understanding of politico-economic environment and its influence on statutory social work delivery.
* Experience of working within a legal framework.
* Experience of implementing care plans and actively working adult protection plans.
* Experience of contributing to the learning of student social workers.
