Generic Assistant Practitioner - Frailty
The role involves delivering compassionate, person-centred care to older adults within a Frailty Unit. The unit provides specialist multidisciplinary care for older people living with frailty, focusing on early assessment, rehabilitation, and safe discharge planning with home first principles. You will work in a collaborative team, benefit from ongoing professional development, and contribute to service improvement initiatives that enhance patient outcomes. You will work under the supervision of registered therapy staff, using evidence-based practice and client-centred principles, to carry out assessment, treatment and discharge within defined parameters and scope of practice. You will liaise with family, carers and other agencies to ensure care is provided in the best possible setting and prefer alternatives to hospital-based care where appropriate.
What we’ll need you to bring
* Experience working in a health or social care environment
* Be a car driver with a full, valid UK/EU/EEA driving licence
* Ability to demonstrate excellent teamwork and the ability to work using your own initiative
* Effective listening, communication and interpersonal skills
* Ability to work with people and as part of a multidisciplinary team
* Excellent time management, planning and organisation skills
* Working knowledge of basic information technology
* Ability to flex to cover a rotating schedule over 7 days including weekends and public holidays
