The North Manchester Early Intervention for Psychosis Team is happy to be able to provide a full-time, permanent care coordinator position. Registered nurses, social workers, and occupational therapists are all encouraged to apply.
There are three teams in the Manchester Early Intervention for Psychosis Service that cover the city: the north, central, and south teams. This job will be in the NORTH team, which is based in Central Park, Northampton Road. However, as part of the backup assessment practitioner rota, you will need to travel around the city and to inpatient units in Greater Manchester as part of the job.
You will work with a group of dedicated and experienced coworkers, and the Clinical Team Manager and Senior Practitioner will be there to help you.
This job is full-time, which means you work 37.5 hours a week. The Team is open from 9 a.m. to 5 p.m. Monday through Friday, but they can talk about flexible work hours.
Main Tasks of the Job – UK Care Coordinator Role
* Overseeing the clinical management of a group of service users aged 14 to 65.
* Complete social care act assessments and go to the Funding Panel.
* Participation in duty cycle – service-wide triage worker, backup assessment practitioner for service-wide assessments, and backup worker for the North team.
* Help and encourage clients to work together on their treatment and recovery plans.
* Encourage clients to help themselves as much as possible, which will help them keep their skills and independence.
* Help promote health and serve as a consultant or advisor to other community services, agencies, and staff members about particular clients and their mental health problems.
* Encourage a culture of learning, education, and professional growth. The team often has student nurses and social workers on placement.
* Follow the Trust Policy and the local reporting procedure for protecting children and vulnerable individuals.
* Take part in s42 investigations to protect people.
* Do specialised mental health evaluations as necessary, as required by the Mental Capacity Act 2007, the Mental Health Act, and the Local Authority Safeguarding Adults Procedures.
* Using the Risk Assessment Tool, you will need to do and write down full risk assessments, make and carry out risk management plans for clients who are at danger of being vulnerable, hurting themselves, or hurting other people physically, sexually, or emotionally.

Education and Qualifications
Core professional qualification, such as nursing, social work, or occupational therapy.
Experience
Experience managing students in a related subject.
Understanding the rules of The Care Programme Approach and Risk Management.
Understand how important it is to protect children and adults who are weak.
Ability to work with both clinical and managerial supervision.
