This role involves providing person-centred care to individuals in their own homes, helping regain their health, well-being, and independence after spending time in hospital. Please note the role is subject to an enhanced DBS check (if not on DBS update Service) and reference verification. Requirements: * Right to work in the UK - Unfortunately, we are unable to offer Tier 2 sponsorship at this time * Full UK driving licence with access to your own vehicle * Transferrable DBS / DBS on the update service would be ideal Working as part of the Reablement team, you will be: * Assisting with personal care (washing, dressing, toileting) * Supporting with medication * Preparing meals, helping with shopping and light household tasks * Providing companionship and emotional support * Liaising and collaborating with Local Authorities/ Discharge teams / District Nurse, and Family members * Excellent time management and administration skills are required due to the nature of the role.
