East Lothian Health and Social Care Partnership is seeking a dedicated, compassionate, and non-judgemental individual who is passionate about supporting people experiencing challenges related to substance use and mental health.
We are excited to offer a newly established role that spans two key areas of need, providing practical, solution-focused support in collaboration with Social Workers within the service. This role is designed to make a meaningful difference in the lives of individuals by offering timely and effective interventions.
The successful candidate will support individuals in the community to achieve their recovery aims as well as support discharges from the Royal Edinburgh Hospital. The postholder will work closely alongside multidisciplinary teams with colleagues in the Community Mental Health Team at East Lothian Community Hospital and the Substance Use Service at the Esk Centre. The focus will be on contributing to early intervention and rehabilitation efforts to achieve positive outcomes, facilitating smooth transitions from hospital to community, and preventing unnecessary admissions.
You will manage a caseload with the support of our Mental Health Senior Practitioner and will often engage in joint working where a Social Worker is already involved.
Additionally, you will have the opportunity to participate in our wider Adult Social Work duty and response system on a rota basis based at John Muir House, gaining valuable experience and contributing to a responsive and dynamic team.
Comprehensive induction and ongoing learning opportunities in both Mental Health and Substance Use will be provided to support your development in the role.

Please Note:
All home/hybrid workers must live in the UK. Where an application for home-working is supported, it will be subject to a satisfactory trial period. Employees will be expected to attend Council offices for the duration of induction, and be on site in East Lothian workplaces, as required. Employees with a hybrid working arrangement in place will be office based and able to work from home, with permission, on an ad hoc basis.
