Overview
We are looking to recruit a motivated and enthusiastic Mental Health Practitioner to join the Mental Health Intensive Support Team (MhIST), an award-winning and forward‑thinking intensive community mental health rehabilitation service. The post is based in Macclesfield. MhIST is a recovery-orientated service that offers specialist assessment, intervention and support to individuals with complex mental health needs across Cheshire & Wirral. Using the principles of dialogical practice, the MhIST multidisciplinary team works alongside patients and their families and social networks, with a focus on meaningful rehabilitation goals, supporting the individual to work towards their vision of recovery.
As a service we value professional development and provide opportunities for ongoing learning, including quarterly team training days and protected reflective spaces. We welcome applicants from a variety of professional disciplines, including Nursing, Occupational Therapy and Social Work.
Responsibilities
* Work intensively with a small cohort of patients experiencing a range of complex mental health needs, providing a high level of value‑oriented care.
* Take a proactive approach in pre-empting crises and supporting patients to remain in the community, with excellent skills in risk management, adaptive care planning and communication.
* Collaborate with a multidisciplinary team to support a holistic and needs‑adapted approach to community rehabilitation.

Induction and Start Dates
New starters commence on one fixed start date each month, in line with the Prepare to Care (P2C) induction programme date, which offers up to 3 weeks of training to meet induction/initial training requirements.
