The Integrated AdvancedNurse Practitioner (ANP) role will work the Substance Misuse Service (ISMS) remotely. This Service provides support, treatment and interventions for people within the criminal justice system who present with drug/alcohol and complex health needs which may be contributing towards the offending issues.


Main duties of the job

* Undertake advanced clinical assessments for substance use
* Initiate, titrate and review Opioid Substitution Therapy (OST)
* Prescribe medication
* Assess withdrawal risk using recognised clinical tools
* Provide harm reduction interventions
* Prescribe within scope of competence and national guidelines
* Monitor medication effectiveness and safety
* Undertake medication reviews
* Ensure safe storage and administration of controlled drugs

Job summary

The ANP role works the Substance Misuse Service (ISMS) remotely, providing remote clinical assessments for people subject to a community court order or with licencee conditions. The ANP will provide clinical assessments, prescribe and deliver ongoing treatment planning for individuals presenting with substance dependency and social vulnerabilities. Working within a multidisciplinary criminal justice partnership, the postholder contributes to therapeutic interventions, harm‑reduction strategies and recovery‑focused care pathways designed to improve health outcomes and support rehabilitation.

The role combines advanced clinical practice with offender health principles, supporting individuals through engagement, stabilisation, treatment, and transition between court‑ordered Drug Rehabilitation Requirements (DRR) through to custody and community services. The ANP plays a key role in reducing drug‑ and alcohol‑related harm, improving wellbeing, and supporting safer reintegration into society through evidence‑based clinical interventions and collaborative working with healthcare, probation, court, prison and third‑sector partners.

The postholder promotes trauma‑informed, person‑centred care and contributes to the overall service objective of addressing the underlying causes of substance‑related offending, protecting individuals, families and communities while improving long‑term recovery outcomes.


Key responsibilities


Clinical practice

* Undertake advanced clinical assessments for substance use
* Initiate, titrate and review Opioid Substitution Therapy (OST)
* Prescribe medication
* Assess withdrawal risk using recognised clinical tools
* Provide harm reduction interventions
* Contribute to multidisciplinary individualised care plans
* Conduct risk assessments including:
o Safeguarding
o Self‑harm risk
* Deliver trauma‑informed and recovery‑oriented care
* Support continuity of care on admission, transfer or release (secure settings)
* Participate in complex case reviews


Prescribing responsibilities

* Prescribe within scope of competence and national guidelines
* Monitor medication effectiveness and safety
* Undertake medication reviews
* Ensure safe storage and administration of controlled drugs


Multidisciplinary working

* Work collaboratively with:
o GPs
o Recovery workers
* Participate in MDT meetings
* Provide specialist clinical advice to colleagues


Leadership & service development

* Support clinical governance and quality improvement
* Contribute to service audits and evaluations
* Mentor junior staff and nursing teams
* Promote best practice in substance misuse care
* Assist with pathway development and innovation


Safeguarding

* Identify safeguarding concerns for adults and children
* Follow statutory safeguarding procedures
* Participate in safeguarding meetings and documentation


Clinical governance

* Report incidents via organisational systems
* Adhere to:
o NICE guidelines


Health promotion

Provide education on substance misuse, harm reduction, and safe medication practices.


Essential qualifications

* Registered Nurse (Adult or Mental Health)
* MRCGP Substance Misuse Part 1 & Part 2
* Current NMC registration
* MSc Advanced Clinical Practice (or working toward)
* Non‑Medical Prescribing Qualification (V300) or willingness to obtain
* Experience in substance misuse or addictions services
* Advanced clinical assessment skills
* Experience managing complex and vulnerable patients
* Dual diagnosis management
* Harm reduction services
* Community addictions services
* Risk management
* Leadership and mentoring


Personal attributes

* Structured and methodical
* Commercially aware and risk‑focused
* Confident leading people and making decisions
* Comfortable escalating risks and raising concerns where necessary
* Ability to work autonomously within an operational environment
