Overview
We are currently seeking a Mental Health Social Worker – North & West to join the Trust! This Band 6 post requires a qualified Social Worker to work within the Adult Mental Health Team, providing statutory social care, conducting assessments, and developing social care plans that meet the needs of service users with complex mental health needs.
Responsibilities
* Identify and address barriers to promoting a social model of mental disorder in partnership with service users, their families, and the multi‑disciplinary AMHT.
* Apply the Mental Capacity Act and Mental Health Act to support service users and safeguard their welfare.
* Articulate the social perspective of mental health and prioritize social care needs within the multi‑disciplinary team.
* Undertake Care Act or S.117 after‑care eligibility assessments for individuals with highly complex needs.
* Intervene and demonstrate professional leadership in situations characterised by high levels of social, family and interpersonal complexity and risk.
* Formulate social care support plans in accordance with the Care Act 2014 and S.117 after‑care eligibility using the Local Authority process.
* Facilitate assessment of carer's needs and implement appropriate support plans.
* Participate in induction, mandatory training and continuous professional development.
* Promote safeguarding, equality, diversity and inclusion throughout all aspects of practice.
Qualifications
* Essential criteria: Qualified Social Worker with statutory social care experience and a deep understanding of mental health legislation.
* Desirable criteria: Experience in Care Act or S.117 after‑care eligibility assessments, demonstrated leadership in high‑complexity situations and knowledge of the Mental Capacity Act.
* Strong communication skills, ability to develop collaborative working relationships and commitment to safeguarding and welfare of children and vulnerable adults.
