The Primary and Urgent Care Alliance is looking for a motivated care coordinator to join our friendly and expanding team.
We are looking for an individual who is confident, skilled and an effective communicator.
We are looking for an individual who can demonstrate experience in using IT systems that are used for coordinating the referrals that come through and someone who is able to work independently and prioritise their work load appropriately.
Main duties of the job
As part of the role, you will be required to triage patient referrals sent in from practices requesting a home visit from our paramedics.

Job description
Job responsibilities
As a care coordinator, your main roles and responsibilities include:
1. Triaging patient referrals,
2. Booking the home visits in on EMIS with the allocated clinician
3. Updating the paramedic's visit sheets
4. Route planning
5. Updating patients on the status of visits
6. Liaising with practices to update on the status of visits
7. Answering calls and assisting with queries from both the practices and paramedics
8. Liaising with the Business Manager as required to provide data
Care coordinators are required to complete online training if not done previously as a requirement of the role. If training is needed this will be funded by the organisation.
