Job Overview
As a core member of the Extended Hope and Extended Hope House Team, you will report to the Service Manager and play a key role in urgent assessments, care planning, and treatment for children and young people with emotional, social, and mental health needs. Your work will directly contribute to improving outcomes for vulnerable youth.
What You Will Do
* Conduct urgent assessments to determine immediate needs of children and young people.
* Develop and implement personalised care plans promoting mental health and emotional wellbeing.
* Work collaboratively with a multidisciplinary team to ensure wrap-around support.
* Engage in reflective practice and contribute ideas to enhance service delivery.
* Maintain accurate documentation and records using SystmOne, ensuring compliance with NHS protocols.
Requirements
* Relevant NHS experience is essential.
* Experience with SystmOne is desirable.
