An exciting opportunity has arisen for a crucial admin role as a Pathway Navigator for patients on Hepatology Cancer Surveillance and will be the central point of contact for the patients on HCC surveillance; helping to ensure the individual needs of the patient are supported and that care is built around those needs.
You will provide direct support for patients to maintain their engagement in the surveillance pathways ensuring that they receive their surveillance scans on schedule. Working closely with the Hepatology CNS and wider Gastroenterology team.
As the central point of contact you will be involved with maintaining both effective communication with and on behalf of the patient, signposting to other resources and information as defined by patient need and liaising with staff and services to ensure tests and appointments are scheduled.
A key element of the role will be to maintain and populate tracking lists for Liver Health Check clinics and to track patients for HCC Surveillance; as such you must have excellent IT and data entry skills.
We operate from three main hospitals – Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal – as well as a number of community healthcare premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant‑led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non‑life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.
Our Community Service for adults is provided in people’s homes, community centres, clinics, GP Practices and community hospitals. Our aim is to work with people to help them remain independent, improve their health and manage their conditions through high‑quality care, advice and support.
