We are looking to recruit and highly skilled, motivated and passionate Band 7 Advanced Nurse Practitioner to join our Adult Community Mental Health Team in Skipton, Craven, providing care to service users in a large geographical area, covering urban and rural communities.
The post holder will provide clinical risk and mental health assessment, formulation, planning and prescribing for individuals with severe and enduring metal health problems.

Main duties of the job
Carry out physical checks prior to the prescribing, initiation and monitoring of medication, and also other medication where necessary. To work with other members of the team to manage and monitor the complex presentations within Craven CMHT.
To prescribe medication within nice guidance, NMC guidance and Local trust policies
To work in partnership with service users, families, and carers to achieve identified goals and to support recovery and reintegration by adopting a fully holistic approach to needs.
To work in a supported autonomous manner and to be able to plan and prioritise workload.
To demonstrate effective networking skills to work in collaboration with a range of other services and external agencies.
To actively participate in the flexible formats of team supervision and support.
To work as part of the leadership team, providing support and cover for team leader when required.

Essential criteria
1. Mental Health Nurse Qualification
2. Independent Nurse Prescriber
3. RN 1st Level
4. To have undertaken accredited courses which enable Advanced Practitioner registration
Desirable criteria
5. Previous experience of working in a community mental health setting
6. Undertaken Advanced Nurse Practitioner course
Experience
Essential criteria
7. Experience of working with people with complex health needs
8. Experience of instigating investigations, interpreting investigations and initiating appropriate plan of care for patients with Mental Ill health
9. Experience of care co – ordination/ care management
Desirable criteria
10. Experience of working in the Community Mental Health care setting

11. Demonstrate a high level of clinical skill. Demonstrate an in depth knowledge and experience of clinical examination, history taking, pharmacology, pathophysiology etc
12. Understanding of NHS Plan and implications for Primary and Secondary Care
