NHSHealth Checks are a systematic prevention programme that assesses anindividuals risk of heart disease, stroke, diabetes and kidney disease, onceevery five years. It is for people aged between 40 and 74 who have not beenpreviously diagnosed with one of the above conditions, hypertension, or arecurrently receiving certain medications.
Theaim of the PCN Heart Health Checks in Surrey Heath Primary Care Network is toprovide a quality service that will help people live longer, healthier lives.The longer term aim is a reduction in incidence or early detection of heartattacks and strokes, type II diabetes, chronic kidney disease and vasculardementia.
As a Health Care Assistant with us, you willplay a key role working under supervision and delivering our heart healthchecks which identifies the persons risk of developing cardiovascular diseaseand diabetes. This health check focus on preventative care, health promotionand education - all within an environment dedicated to providing respectful,high-quality service to every patient. You will have excellent communicationskills and will understand the importance of communicating tailored advice andsupport.
Main duties of the job
To undertake NHS health checks
Phlebotomy every patient will have a blood test as part of the hearthealth check
Carry out baseline observations such as pulse oximetry, blood pressure,temperature, pulse rate, recording findings accurately
Accurately measure and record clinical measurements such as height,weight and waist measurements
Ensuretimely and accurate recording of clinical information and patient interventions on relevantelectronic database systems, e.g. EMIS, and following business continuityplans if the electronic system is unavailable
Communicateeffectively with patients and carers, recognising the need for alternativemethods of communication to overcome different levels of understanding,cultural background and preferred ways of communicating.
Provideand communicate relevant health promotion literature and lifestyle adviceto patients
Ensureall clinical rooms are adequately stocked and prepared for each session
Ensureclinical waste is removed from clinical areas and sharps bins replaced inaccordance with IPC policy
Work asan effective and responsible team member, supporting others and exploringmechanisms to develop new ways of working.
Supportthe PCN with health promotion programmes as appropriate
To carryout any other reasonable duties as requested by a manager to ensure quality ofservice

Personal Attributes

Qualifications
* GCSE Maths and English (C and above) or equivalent.
* Six months of relevant experience working in a primary care environment in the last two years.
* Care Certificate.
* Relevant NVQ Level 3 qualification or equivalent.
Knowledge
* Experience of working with healthcare professionals and/or previous experience in the NHS or social care.
* Experience coordinating with multiple stakeholder or individuals to meet specified outcomes.
* Experience providing advice/signposting.
* Experience using a patient clinical system such as EMIS.
* Knowledge of safeguarding interventions and an awareness of the Mental Capacity Act.
* Knowledge of a range of local community groups which support wellbeing.
Skills
* Excellent verbal and written communication skills with the ability to communicate effectively at all levels including with patients, carers, specialist services, GPs and colleagues.
* Good technical literacy with e.g. Word, Excel, and experience using a clinical system such as EMIS.
* Able to work independently and manage own workload.
* Able to build strong professional relationships.
* Understanding of social determinants of health and how these can be addressed with patients.
