Post: Registered Nurse (Non-Medical Prescriber) Hours: 15 Hours per week Fixed term contract for 12 months Service Operates: Monday to Friday between 0800-1830 Pay: £23.61 - £27.01 depending on experience Reports to: Clinical Manager Accountable to: Head of Primary and Community Care Base: Newfield House, Blackpool undertaking Clinics across Blackpool, Fylde and Wyre. Job Summary We are looking for individuals with a passion and enthusiasm to work with some of the most vulnerable and complex people within our community. The right candidates will be approachable and display excellent communication skills, compassion, tolerance, and a non-judgemental attitude. This is an exciting opportunity to work in new and innovative ways, positively impacting on the lives of people within the homeless community. As part of this service, we are looking for nurses who have a passion for respiratory to undertake lung function testing on patients at high risk of respiratory conditions including COPD, Asthma to those that may require a fast track for further investigations. Duties and Responsibilities To provide clinical skills and advice demonstrating a sound understanding of the issues related to identification, assessment, diagnosis, treatment, and management of the client group within scope of practice. To provide respiratory reviews for patients including lung function testing when appropriate When applicable, participate and contribute to the multi-disciplinary team for homeless people. Input into holistic care plans, to ensure a focus on the individuals goals and support them to comply with the plan ultimately improving health outcomes, Increase health literacy and co-operation with health prevention and treatment pathways for homeless people Provide outreach to individuals who cannot attend a designated location. The role will involve working across different sites and will include clinics for homeless people on the health bus Work to attain the service KPIs and target Ensure all patient contact is recorded in a timely manner on the IT systems to enable tracking of patient outcomes, both in real-time to support ongoing intervention planning and retrospectively to understand the impact of the service To ensure mandatory training is kept up to date Acknowledge and accommodate barriers to communication and understanding including speech, hearing, language, and emotion. Initiate emergency action when required Obtain and dispatch specimens on request Act in accordance with the NMC Code of Professional Conduct Produce accurate, contemporaneous, and complete records of patient consultation, consistent with legislation, policies, and procedures Prioritise, organise, and manage own workload in a manner that maintains and promotes quality Deliver care according to local and national guidelines Obtain and document consent for any treatment undertaken and keep the patient at the centre of his/her own care, ensuring full participation in decision making Work within all relevant policies and procedural guidelines e.g., infection control, chaperoning and risk management Safeguarding responsibilities for adults and vulnerable groups, reporting concerns appropriately at the correct time and engaging with services as needed. The Head of nursing may also ask for other duties to be carried out to the benefit of the organisation. It is made of 3 areas of practice: -Compass Medical Practice this a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted with via the telephone /video link and when a face-to-face consultation is required a risk assessment is undertaken to see what support the clinician requires. Consultations take place with a patient liaison officer in wither our mobile van or in an agreed location pre booked in advance. -Homeless Health Team - FCMS provides a healthcare service for Homeless People within our local community. Our team of clinicians are co-ordinating the care of the homeless population and offering enhanced support for those who are not accessing mainstream services. The team run clinics around the local town centre, working closely with other agencies that support this community. The team offer a comprehensive and holistic assessment of the individuals needs and work with existing health services to meet these and use every contact opportunity to offer health promotion and improve the health outcomes for these patients. The team offer everything from wound care; immunisations; management of long-term condition advice, to supporting with complex care needs. -Asylum Seeker Clinics we provide new patient health checks to these patients whilst registering them at a local GP practice at the same time. This takes place in the hotel where they are staying with the aid of interpreters. Educational To follow safe working practices and to always comply with local Safety Policies and Procedures To ensure confidentiality on all matters relating to patients and information obtained during employment and not to release such information to anyone other than those acting in an official capacity as instructed Maintain good relationships with other members of the staff team across all disciplines Contribute to the identification of own training needs and participate in relevant in-service training Where necessary attend relevant training in the operation of new or unfamiliar equipment, software or procedures will be provided or arranged. Confidentiality In the course of your duties, you may have access to confidential information about patients, staff or health service business. On no account must such information be divulged to anyone who is not authorised to receive it. Confidentiality of information must be always preserved whether at or away from work. FCMS has in place a Whistle blowers Policy for staff wishing to express concerns. Data Protection Act, 1998 Carry out any requirements within the duties applicable to the Data Protection Act, 1998. Outside Employment/Outside Interests If you have other work or outside interests, this must not conflict with your duties and responsibilities of your attendance for work as an employee of the Practice. It is a condition of appointment that you must inform your manager before taking up of any private practice, work for outside agencies or other employers, other work for this Practice (including bank work), voluntary work or outside interests you have or propose to have. This is to ensure there is no question of it creating a conflict of interest with your NHS duties. You must also therefore seek your manager's approval before taking on any such other work or outside interest at any time after entering employment. Working Time Directive You are required to comply with the regulations governing working time and to any locally agreed associated arrangements. The role is subject to a DBS check which will require three forms of valid ID to be produced and verified. Experience of caseload holding and multi-disciplinary working Desirable Experience working with vulnerable or complex groups of patients Long term condition management experience Health promotion Experience Qualifications - Essential Good standard of general education Registered Nurse (NMC) with relevant experience post qualification At least 2 years post qualification Accredited spirometry qualification including interpretation of results. NMP V300 qualification Respiratory qualification Desirable Be able to undertake FENO testing.
