Advanced Clinical Practitioner or Advanced Nurse Practitioner
Main duties of the job:
* Utilise your skills in managing patients within the urgent treatment care setting.
* Act as an autonomous advanced practitioner, working independently and with the clinical team.
* Demonstrate highly developed clinical skills, critical thinking, and work within professional boundaries.
* Collaborate with GPs, Urgent Care Practitioners, Nurse Streamers, and Health Care Assistants to deliver excellent care within UTC.
* Make safe, competent clinical decisions, including assessment, diagnostics, treatment, and evaluation.
* Communicate effectively across all levels and work closely with the multidisciplinary team.
* Complete streaming, assess patients independently, prescribe interventions, and handle administrative tasks such as prescriptions and referrals.
* Provide high-quality, person-centred, holistic care to patients.
Clinical Practice:
* Assess, diagnose, plan, implement, and evaluate treatments for undifferentiated and complex cases.
* Identify and manage long-term conditions and both acute and chronic illnesses.
* Prescribe and review medications, supporting patient adherence and providing medication advice.
* Prioritise health problems, including urgent and emergency interventions.
* Support health promotion and self-care strategies.
* Refer patients with mental health needs as per guidelines.
* Deliver evidence-based care for aural and wound care needs.
Communication:
* Use sensitive communication styles to ensure informed consent.
* Adapt communication methods to meet diverse patient needs and overcome barriers.
* Maintain effective communication within the practice and with external stakeholders.
* Advocate for patients and colleagues.
Delivering a quality service:
* Work within own competence and professional codes (e.g., NMC, HCPC).
* Maintain accurate, complete records in accordance with legislation and policies.
* Manage workload to promote quality care.
* Deliver care aligned with NSF, NICE guidelines, and evidence-based practices.
* Evaluate care effectiveness through review and benchmarking.
* Participate in shared learning and quality improvement initiatives.
* Recognise legal issues related to vulnerable children and adults, following statutory procedures.

* Support clinical development of students as needed.
* Promote the organisation's vision, values, and goals.

* Support service improvements and patient safety initiatives.
