Overview
The Community Navigator will support the administration provision for the Intermediate Care (Hospital@Home) Service, ensuring duties are maintained and delivered efficiently and effectively.
Key Responsibilities
* Utilise a variety of IT functions and systems used in the Intermediate Care Service and maintain wide knowledge of clinical and non‑clinical information systems.
* Communicate effectively with the service, multidisciplinary team, patients and their relatives, and signpost individuals to the appropriate service when necessary.
* Be knowledgeable in Cwm Taf Morgannwg University Health Board policies and procedures.
* Provide cross cover for other Community Navigators during times of absence.
Language Requirement
Welsh to be learned on appointment. The post is advertised as 'Welsh to be learnt on appointment' and we will provide training if you do not already have Welsh skills.
