Overview

Job summary: Provide the clinical pharmacy services to a local hospice via SLA; CF patients in collaboration with the CF pharmacy team and wider CF MDT; manage, monitor and develop the clinical pharmacy service for homecare initiatives.


Responsibilities

* Provide and support the delivery of high quality pharmaceutical care to meet the needs of palliative care patients within the local hospice settings under service level agreements, as well as LHCH patients (cross-site).
* Provide a clinical pharmacist role for medical staff and nursing staff on drug use in palliative care within the hospice environment.
* To work with palliative care practitioners and other professionals to facilitate the delivery of national and local health policy.
* To participate in the palliative care education and training programmes of healthcare professionals and students under the guidance of the Senior Clinical Pharmacist - Palliative Care / Clinical Trials.
* To be responsible for planning and reviewing the security of medicines at the local hospices, under the direction of the senior clinical pharmacist for palliative care and Deputy Chief pharmacist for clinical services, and to monitor their drug expenditure on a monthly basis with annual reviews, maintaining full control of such.
* To provide a clinical pharmacy service to the CF patients in collaboration with the CF pharmacy team and wider CF MDT.
* Under the direction of the Deputy Chief Pharmacist, manage, monitor and develop the clinical pharmacy service for homecare initiatives.
* To provide a clinical pharmacy service to the CF patients in collaboration with the CF pharmacy team and wider CF MDT.
* To attend multidisciplinary team meetings, ward rounds and CF outpatient clinics as appropriate.
* To conduct annual reviews and provide discharge information to Community Pharmacists and General Practitioners as required.
* To support the provision of a specialised clinical pharmacy service to Cystic Fibrosis patients.
* To participate in out-of-hours, bank holidays and weekend provision of pharmaceutical services and, in doing so, be solely responsible for the management of the service at these times, including the safety and security of the pharmacy department and all its contents (including medicines within controlled storage areas such as refrigeration and Controlled Drugs).
* To cover the duties of other pharmacists during periods of sickness and absence and undertake any other reasonable duties required.
* To provide a highly specialist clinical pharmacy service to LHCH patients via SLAs (off-site) and attend weekly multidisciplinary ward rounds.
* Provide clinical and medicines information to support the work of the team and establish communication with Primary Care professionals to facilitate transfer of patient care when necessary.
* Provide a clinical pharmacist support role to all Hospice patients and attend meetings at the request of the Chief Pharmacist.
* Offer advice to staff and the general public within the guidelines of the standard operating procedures for the sale of medicines from the pharmacy retail area.


Managerial/Leadership

* To support and develop the annual Pharmacy Business Plan.
* To interpret national legislation, policy or guidance applicable to the specialty and develop and implement pharmaceutical guidelines and/or policies in conjunction with the Senior Clinical Pharmacist Palliative care / Clinical Trials.
* Review NICE Guidance in relation to palliative care and incorporate recommendations into prescribing policies and drug formulary following Drug and Therapeutics Committee review.
* Plan, develop and implement prescribing guidelines and protocols for the palliative care team in conjunction with the Senior Clinical Pharmacist - Palliative Care / Clinical Trials.
* Review and update policies/procedures to ensure compliance and best practice for homecare services; liaise with regional homecare service lead and action relevant communications.
* Review homecare KPIs in conjunction with the Deputy Chief Pharmacist (patient services).


Qualifications & Skills

Essential

* Vocational Masters Degree in Pharmacy or equivalent
* Registered member of the General Pharmaceutical Council
* Post-graduate Diploma / MSc in Clinical pharmacy

Desirable

* Non-Medical Prescribing certificate - or ability to work towards

Skills & Knowledge

* Able to accept responsibility for a specialist area of pharmacy practice - previous demonstrable skill
* Able to work independently and compute information from various sources quickly
* Excellent communication skills, both verbal and written
* Ability to work on own initiative and as part of a team
* Good time management skills

Desirable

* Previous experience in providing pharmaceutical care to palliative care patients
* Previous experience in providing pharmaceutical care to cystic fibrosis patients
