This includes providing care and support to individuals living with dementia, mental/physical illness, learning disability and those who require support within a reablement model to return home where possible.You will be responsible, as part of a multidisciplinary team, for the provision of high-quality person-centred care for each individual. You must have knowledge and experience of good care practice in meeting the needs/outcomes of the individuals we support. You will be able to demonstrate skills in assessment and recording and should be able to communicate effectively and have a flexible and positive approach.An SVQ 3 in Social Care is desirable and you must be able to register with the SSSC and qualify within the regulatory timescale years.There are a number of casual social care worker posts, where the successful candidates would backfill shifts on days and/or nights on an as required basis.
