We are seeking a triage clinician, who can manage all same day urgent treatment requests. Working within your professional boundariesyou will provide care for patients from initial history taking, clinicalassessment, diagnosis, treatment and evaluation of their care.
Main duties of the job
The post holder will, under appropriate supervision:
1. Provide first point of contact for patients presenting with undifferentiated, un-diagnosed problems, utilising history taking, physical examination, problem-solving and clinical decision-making skills to establish a working diagnosis and management plan working in partnership with patients.
2. Undertake surgery based consultations for emergency or routine problems. In addition, undertake telephone and video consultations, and home visits as required.
3. Be confident to assess patients of all ages, including paediatrics.

Ourfriendly rural GP Practice in the Cotswolds includes two GP partners and two salaried GPs. Our nursing team comprises of two HCA's and two nurses.
We have a list size of 5,500, which is growing.
We have access to a clinical pharmacist, mental health nurse, care co-ordinators, a frailty task force and social prescribers via our PCN.

ANNUAL LEAVE: 33 days including bank holidays (full time workers) pro rata for part time workers
This role is salaried by the practice not under ARRS funding.

Duties and Responsibilities

4. Provide first point of contact for patients presenting with undifferentiated, undiagnosed problems, utilising history taking, physical examination, problem-solving and clinical decision-making skills to establish a working diagnosis and management plan working in partnership with patients.
5. Undertake surgery based consultations for emergency or routine problems. In addition, undertake telephone and video consultations, and home visits as required.
6. Instigate necessary diagnostic tests or investigations and interpret findings/reports within the scope of your practice. Discuss the result and implications of laboratory investigations with patients
7. Be confident to assess patients of all ages, including paediatrics
8. Utilise clinical guidelines and promote evidence-based practice.
9. Offer a holistic service to patients and their families, developing where appropriate an on-going plan of care/support with an emphasis on prevention and self-care.
10. Refer patients directly to other services or agencies using appropriate referral pathways
11. Ensure safe handover of care within and outside the practice as appropriate
12. Work directly with members of the practice primary health care team and support integrated patient centred care through appropriate working with wider primary care / social care networks
13. Identify community health needs and participate in the development of patient/family-centred strategies to address them.
14. Contribute to the practice quality targets to consistently achieve high standards of safe, evidence-based, cost-effective patient care and service delivery
15. Prescribe medication and work within the limitations of current extended Independent Prescribing legislation.
16. Demonstrate safe, clinical decision-making and expert care for patients within the general practice. You will work with the multi-disciplinary general practice team to support the delivery of policy and procedures and meet the needs of patients.
