Our Client, Reading Council is seeking an Adult’s Social Worker to join their Physical Disability and Older People Team.

Responsibilities of this role:
* Evaluating the physical, social, emotional, and cognitive needs of individuals with physical disabilities and older adults.
* Developing individualized care plans that address identified needs, promoting independence, and maximizing choices.
* Providing a range of supports, including practical assistance with daily living, equipment, home adaptations, and access to community resources.
* Working collaboratively with individuals, families, carers, healthcare professionals, and other agencies to ensure holistic and coordinated care.
* Advocating for the rights and needs of individuals with disabilities and older adults, ensuring they have a voice and control over their lives.
* Identifying and addressing any safeguarding concerns, ensuring the safety and well-being of individuals.
* Working with individuals who may be experiencing mental health challenges, such as depression or anxiety, and coordinating care with mental health professionals.
* Working with complex cases, including those with multiple health conditions, social issues, or legal matters.
* Conducting Mental Capacity Act assessments to ensure that individuals have the capacity to make their own decisions about their care.
Requirements of this role:
* Must be a Qualified Social Worker, registered with Social Work England.
* Must have minimum of 3 years post qualifying social work experience
