What youll be doing Supporting individuals with complex needs, including mental and physical health challenges Managing a caseload and creating personalised support and move-on plans Working closely with partner agencies to deliver holistic, wraparound care What were looking for At least 6 months experience working face-to-face with vulnerable individuals Understanding of homelessness, trauma, and complex needs Strong communication skills, with the ability to build trust and motivate others A DBS on the Update Service, or willingness to obtain one Why this role?
