Job Role: Care Team Leader
Hours: Full time shared between 2 days a week in the office and front-line care.

Candlelight Care are looking to recruit a new Team Leader for the Wimborne branch.
Job Purpose:
This is an exciting and varied role, which will include being responsible for evaluating and assessing new client packages, developing care plans, carrying out risk assessments, conducting reviews of existing packages, liaising with the Registered Manager and clients, evaluating care progress and recommending changes to the care plan if required. Supporting our team of Care & Support Workers. Carrying out staff observations, and supervisions.
You will also be part of the on-call team on a rotational basis.
Role Responsibilities:
As the role can change on a daily basis, you will need to be highly organised, efficient and flexible in your approach.
Carrying out of assessments of all new packages.
Meet and greet with our potential private clients and/or their families.
Re-assess existing packages, for example, when a client is discharged from hospital with a change in their needs, etc.
Carrying out required risk assessments to ensure delivery of safe care.
Updating care plans, and changes of medication and other care related documents found within the client's electronic care plan and care folder, as appropriate.
Undertaking client reviews under the direction of the Registered Manager.
Undertaking of staff observations and/or supervisions under the direction of the Registered Manager.
Liaise with other health and social care professionals.
Undertake care work, when required to meet the needs of the business.
Working hours to include a minimum of two evenings and an alternate weekend.
Be part of the on-call rota, ready to cover any outstanding care visits at very short notice.
Available to attend Team Leader Meetings on a regular basis

Previous experience within a care role.
Good working knowledge of care and support work.
Understanding and respecting the need for confidentiality
Diploma level 2 in Health & Social Care or equivalent
Good communication skills
Ability to work without supervision
Caring and empathetic approach
Reliable and trustworthy
Ability to manage your own time
Good team player, who is understanding and supportive of colleagues
A full (clean) driving licence and access to own transport.
