What You’ll Do
The Advanced Nurse Practitioner/Advanced Clinical Practitioner plays a critical role in supporting virtual care by providing support and advice to our remote monitoring clinicians. The post holder will work as an experienced, autonomous practitioner who, acting within professional boundaries, will provide care for patients remotely and from history taking, clinical assessment, diagnosis, treatment and evaluation of care. You must demonstrate critical thinking in the clinical decision‑making process to ensure safe and expert You will use the web‑based Doccla platform to monitor patient wellbeing, support their care needs and assist in delivering high‑quality care in collaboration with the wider clinical team.
* Deliver high‑standard, autonomous care in primary care settings through advanced clinical skills and theoretical knowledge
* Complete remote patient consultations via telephone and video
* Support the remote monitoring clinicians when required, assisting the monitoring of vital signs and collection of patient data through remote technology
* Assess, diagnose, plan and evaluate care, ensuring timely interventions for acute, undifferentiated conditions
* Investigate necessary invasive and non‑invasive diagnostic tests or investigations and interpret findings and reports
* Refer patients to primary and secondary care or specialists when required
* Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
* Prescribe medication safely and effectively in accordance with the latest legislative framework, and provide information on medications and side effects
* Take responsibility for making autonomous decisions in relation to patient care
* Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long‑term condition (as appropriate)
* Diagnose and manage acute conditions, integrating both drug‑ and nondrug‑based treatment methods into a care plan
* Work with patients to support compliance with and adherence to prescribed treatments, providing information and advice on medication regimens, side‑effects and interactions
* Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
* Encourage and support patients with long‑term conditions to develop their ability to self‑manage
* Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures on the clinical system
* Provide support to patients in their home environment, ensuring their comfort, safety and wellbeing
* Communicate effectively with patients, offering reassurance and advice as directed by clinical protocols
* Escalate patient concerns to the appropriate healthcare professionals as per established protocols
* Participate in clinical audits and quality improvement initiatives
* Provide general administrative support to the clinical team, including managing appointments and patient follow‑ups
* Support the integration of care pathways for remote patient monitoring to enhance patient outcomes
What We’re Looking For
Qualifications
* Registered with a UK professional body (e.g., NMC, HCPC)
* Degree‑level qualification in nursing or equivalent post‑registration award
* Non‑Medical Prescribing qualification
* Advanced clinical practice experience
* Experience working in Primary Care, with significant post‑registration experience
* Ability to practice within professional boundaries and adhere to regulatory standards
* Current Care Certificate
* Right to work in the UK
Desirable
* Master’s in Advanced Clinical Practice
* Experience in remote patient monitoring or virtual wards
* Familiarity with digital healthcare platforms
* Experience working in a start‑up or rapidly changing environment
