We are a dynamic,forward thinking PCN team who are passionate about developing and deliveringexcellent quality local services to meet the needs of our patients. Due to thechanges in the ARRS Scheme to introduce General Practitioners, we are looking toexpand our team by employing a newly qualified General Practitioner towork alongside our practices and PCN staff (4 sessions per week). Our PCN staff currently includesClinical Pharmacists, Physician Associates, Social Prescribers, Paramedics,Physiotherapists, Care Coordinators, Nurse Associates and a Health andWellbeing Coach.


Main duties of the job

To perform as anautonomous General Practitioner working independently and with other healthcare professionals to assess, diagnose and treat the conditions of patientsattending within a Primary Care setting. Provide expert professional advice topatients, carers and colleagues and ensure the maintenance of clinicalexcellence. To be involved in the provision of education and training to GPRegistrars and other staff. To be professionally accountable and responsiblefor all elements of clinical practice in accordance with the GMC code ofconduct ensuring the safety of patients and quality of their care is the primaryfocus. The post holder will need to becapable of independent travel across Harpenden Health PCN and work flexiblyto meet the needs of the Business.

Job responsibilities

Main Duties andResponsibilities

Clinical Responsibilities

In accordance withthe practice rota, as agreed, the post-holder will make themselves available toundertake a variety of duties including surgery consultations, telephoneconsultations and queries, visiting patients at home, checking andsigning/authorising repeat prescriptions and dealing with queries, paperwork,and correspondence in a timely fashion.

Making professional,autonomous decisions in relation to presenting problems, whether self-referredor referred from other health care workers within the organisation. Assessingthe health care needs of patients with undifferentiated and undiagnosedproblems.

Screening patientsfor disease risk factors and early signs of illness.

In consultation withpatients and in line with current practice disease management protocols,developing care plans for health.

Providing counsellingand health education.

Admitting ordischarging patients to and from the caseload and referring to other careproviders as appropriate.

Recording clear andcontemporaneous consultation notes to agreed standards.

Compiling and issuingcomputer-generated acute and repeat prescriptions.

Prescribing inaccordance with the practice prescribing formulary (or generically) wheneverthis is clinically appropriate.

The post-holder willbe required to visit patients during surgery time in an emergency if clinicallyindicated.

The post-holder willundertake a fair share of On Call sessions per week as allocated on thepractice rota.

The post-holder willundertake fair share of home visiting sessions per week as allocated.

The post-holder willundertake PCN Clinical work and support projects on e.g. Frailty.

The post-holder willundertake work supporting local Care Homes.

The post-holder willsupport work towards achieving QOF, ECF and PCN DES targets.

The post-holder willbe required to support our Enhanced Access rota.

Training/Meetings

Attending trainingand events organised by the practice or other agencies, where appropriate.

Participatepositively and actively in clinical and practice meetings.

The PCN and Practiceshave a strong ethos around training and development of others, therefore thesuccessful candidate will be willing and able to support medical students, andother training roles.

The post holder will be assigned a Clinical Supervisor to support themin their role.

Create opportunitiesto address patients care needs in the most expedient way, taking into accountthat continuity of care and access to services are often in direct conflict.

Encourage patients tobe aware of the service pressures within primary care and to manage theirexpectations of what we can realistically deliver in a manner that is fair andproportionate for all care priorities.

Make improved use ofthe telephone and other technologies. Encourage patients to rely on faceto-face consultation only where there is a clinical need to do so.

Communicate andreassure patients of the need to offer alternative care options whereappropriate and safe to do so.

To immediatelyhighlight any concerns you may have in respect of safety or quality of patientcare to the Clinical Director/Practice Partners/PCN Manager.

Manageself/Professional Development:

Develop and maintainan up to date personal development plan and professional portfolio for purposesof annual individual performance review, appraisal, and revalidation.

Ability to developown knowledge and clinical practice, meeting objectives and any changes inservice.

Manage own time, workload,and resources effectively.

To provide educationand training to other staff and Registrars as appropriate.

A commitment tolife-long learning and audit to ensure evidence-based best practice.

Awareness of andcompliance with all relevant practice policies/guidelines, e.g. prescribing,confidentiality, data protection, health and safety.

Manage and use Information

Makes accurate recordkeeping in EMIS and makes effective use of the clinical system by ensuring theconsistent and accurate entering of data and use of Snomed Codes.

Facilitates the useof up-to-date information technology in order to collate accurate and timelyinformation as and when required by the practice.

Works within theCaldicott and Data Protection principles.

Alert other teammembers to issues of quality and risk.

General

Person Specification


Experience

* Experience of working in a primary care environment.
* Experience of continued professional development.
* Newly Qualified General Practitioner (with under 2 years from qualification).
* Not previously been substantively employed at a GP Practice.
* General understanding of the primary care contract.
* Experience of ICB initiatives.


Skills and Personal Qualities

* Excellent communication skills (written and oral).
* Clear, polite telephone manner.
* Competent in the use of MS Office and Outlook.

Qualifications

* Vocational Training Certificate or equivalent JCPTGP.
* General Practitioner (Certificate of Completion of Training CCT).
* Full GMC registration.
* National Performers List registration (England).
* Appropriate defence indemnity (MPS/MDU).
* Eligibility to practice in the UK independently.
