Social Worker - Adults Multi-Disciplinary Team
Salary Up to £68,181 per annum depending on experience
Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for an Adult Team Manager within the Hospital Team to work full time based in Redbridge.
The salary for this Adult Team Manager job is up to £68,181 per annum.
* Provide leadership, management, and professional supervision to a multi-disciplinary hospital social work team.
* Ensure staff deliver high-quality assessments and interventions in accordance with statutory requirements.
* Manage performance, workload allocation, and service delivery.
* Support staff development through supervision, appraisal and training.
* Oversee assessments and discharge planning for adults admitted to hospital.
* Ensure compliance with Discharge to Assess pathways and safe discharge processes.
* Promote timely discharge to prevent unnecessary hospital stays and improve patient outcomes.
* Monitor and manage complex hospital discharge cases involving safeguarding or high risk.
* Work collaboratively with hospital multi-disciplinary teams including doctors, nurses, therapists and discharge coordinators.
* Deputise for the head of service and keep them apprised of sensitive, high-profile issues.
* Support the Head of Service to engage with stakeholders and commissioners to support service design, innovation, problem-solving and development.
* Ensure a high-quality, high-performing and effective health and social care service is delivered and responds in a timely manner, within statutory guidelines, policy and procedures.
* Develop strong partnerships with NHS trusts, community services, housing providers, and voluntary organisations.
* Monitor service performance indicators relating to hospital discharge and delayed transfers of care.
* Conduct audits of practice and provide feedback to maintain high standards of service delivery.
* Contribute to service development initiatives to improve hospital discharge pathways.
* Implement national policy changes affecting hospital discharge and adult social care services.
* The above mentioned duties are neither exclusive nor exhaustive. From time to time you may be required to undertake responsibilities outside the normal remit of your JD as required by the line manager, and are broadly within you’re the grading level and competence.
Requirements of this Adult Team Manager
* A social Work Qualification or equivalent.
* Understanding of relevant legislation.
* Registered or eligible for registration.
