New and exciting opportunity for a Locality Hub Coordinator to support the delivery of clinical services within a Locality Hub model, a key component in achieving Neighbourhood Health as outlined in the NHS 10 Year Plan. The Birmingham Community Health Company (BCHC) leads the Community Care Collaborative, recognised nationally for its collaborative approach across health and care sectors.
There are five designated Hubs across Birmingham (North, South, West, Central, East). The East Locality Hub (Washwood Heath Health Centre) has operated for over 12 months and has established the framework for delivering neighbourhood health across the remaining four localities, all planned to launch in winter 2025. Core components of the Hub include care coordination, Integrated Neighbourhood Teams, and designated clinical pathways to provide same‑day emergency care and prevent unnecessary admissions.
Key Responsibilities
* Support clinical teams in managing referral pathways across primary, community, and acute care to ensure patients are triaged and streamed into appropriate services within the locality:
o Community Nursing Services
o Pathway 1 – Early Intervention Community Team
o Pathway 2 – Intermediate Care Inpatient Beds
o Pathway 3 – Referrals for care home settings when ‘Home First’ has been excluded
o End of Life (EOL) – Fast Track processes and outcomes
o Integrated Neighbourhood Team (INT)
* Liaise with community multidisciplinary team (MDT) leads to coordinate attendance from community teams to support complex discharge planning and high intensity service user (HISU) pathways.
* Coordinate the Gold Standards Framework (GSF) / palliative care meetings for identified Palliative/End of Life fast‑track patients.
* Support the review of patients referred for Pathway 2 by working with the In‑Reach team and other clinicians to ensure decisions are appropriate to the patient’s current circumstances and admission criteria.
