We are seeking an enthusiastic, self-motivated, and well-organised Home Visiting Practitioner to work 22.5 hours per week over 3 days within our Primary Care Network Home Visiting Service. You will be a dedicated professional who shares our values and passion for delivering high-quality, joined-up clinical management of patients in their own homes. You will have the clinical expertise and ability to autonomously assess and treat patients with a range of conditions and be comfortable acting independently and seeing patients without prior contact with their GP. You will have experience in primary care working and be confident in formulating rapid and accurate treatment plans.
Mid Furness Primary Care Network consists of 4 practices serving a population of 30,000 patients. Mid Furness PCN is a forward-thinking Primary Care Network.
