We are seeking a dedicated Community Support Worker to become part of our multi-disciplinary team as we expand our services.
In line with the NHS Long Term Plan, Lincolnshire was selected as one of 12 areas to enhance community mental health services. Our Community Rehabilitation Team is innovating new approaches for individuals with complex trauma, psychosis, and enduring mental health conditions. We have recently expanded our services in Lincoln, Gainsborough, and now include Grantham and surrounding areas. As part of this exciting transformation, we aim to strengthen our team, which includes Community Support Workers, Occupational Therapists, Nurses, Social Workers, and Psychiatrists.
We collaborate with housing providers, CMHTs, and third-sector organizations to support recovery and wellbeing.

Responsibilities include working within the policies and procedures of Lincolnshire Partnership NHS Foundation Trust, liaising with team members, working independently when necessary, promoting equality, understanding service user perspectives, maintaining accurate records, participating in supervision and training, and working flexibly across a 7-day service.
You will also facilitate recreational and physical activities to support health and social care.
