This role is part of a Multi‑Disciplinary Team dedicated to the assessment, monitoring and treatment of patients with Glaucoma. The post holder will see patients in nurse‑led outpatient clinics and act as a point of contact for existing and newly diagnosed glaucoma patients.</p><p>Key responsibilities include:</p><ul><li>Conduct specialist assessment of patients nursing needs, plan, implement, and evaluate care delivery using advanced clinical knowledge and skills.</li><li>Lead clinical care by managing a patient caseload, providing expert assessment, planning, and evaluation.</li><li>Co‑ordinate and manage nurse/AHP‑led clinics within the speciality.</li><li>Contribute to a multi‑professional approach to patient management, including ongoing care needs and discharge arrangements.</li><li>Establish and maintain effective communication with patients, carers, and relatives.</li><li>Support patients and families, ensuring they receive the required information to enable self‑management.</li><li>Recognise changes in patient condition requiring intervention by others and refer appropriately.</li><li>Assist and encourage the multi‑disciplinary team in resolving patient problems, providing innovative models of case management.</li><li>Lead the specialist nursing contribution to the service, working in partnership with lead clinicians, General Managers, and Matron to ensure high standards of patient care.</li><li>Promote the development of nurses/AHPs undertaking the CNO ‘Ten Key Roles’, where appropriate (e.g., prescribing).</li></ul><p>Requirements:</p><ul><li>At least 2 years of experience working within an Ophthalmic setting.</li><li>Relevant Ophthalmic Nurse Specialist qualification.</li><li>Band 6 competency or an ANEX 21 training post (75% Band 6 pay until competence achieved).</li><li>Ability to act as a deputy for the Band 7 manager.</li></ul></div>
