Overview
This is an exciting opportunity to join the Wrexham Learning Disability Community Health team at Betsi Cadwaladr University Health Board. This is a Band 6 Community Learning Disability Nurse position within Wrexham. The post holder will work as part of the Learning Disability team based at Wrexham, ensuring interfaces between the Local Authority and neighbouring services within the Division including community mental health teams, in-patient services, as well as other statutory agencies and the voluntary sector. The post holder will deliver evidence based, safe, effective interventions and clinical care, following the nursing process.
Responsibilities
* The post holder will follow the service principles and strategy for people with a learning disability while delivering safe, effective and high-quality specialist service in line with agreed standards.
* The post holder will have excellent communication skills and be a visible and accessible member of the team; patients and their families can access them for assistance, advice and support.
* The post holder will communicate and receive highly complex and sensitive information, analyse and interpret information, and act appropriately where there may be barriers to acceptance.
* The post holder will have a good understanding of the CHC process and be involved in the development of pathways and the Maturity Matrix.
* Welsh language skills are desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Essential criteria
* RNLD Qualification
* Evidence of a degree level qualification or equivalent in line with NMC registration
* Evidence of post graduate education and ongoing CPD
Desirable criteria
* Management qualification and/or management experience
* Mentor/supervisor/assessor
Experience
Essential criteria
* Significant amount of experience within the clinical speciality or a relevant nursing role
Desirable criteria
* Experience of working in a multidisciplinary team setting
* Experience/knowledge of senior nurse role
