Job Summary
Advanced Pharmacist Acute Care role at NHS. You will be the Advanced Pharmacist for the Acute Care wards and will support the development of the clinical pharmacy service to wards within acute care, including the Emergency Department, Medical and Surgical Admissions Units, Short Stay Unit and the Ambulatory Emergency Care Unit in line with local and national standards and policies, ensuring the safe, clinical and cost‑effective use of medicines and staff resources.
You will provide a highly specialised level of pharmaceutical care to patients and will support other healthcare staff in meeting risk management, financial management and clinical governance responsibilities around the use of medicines.
You will provide highly specialised advice for pharmacists and healthcare professionals, including participation in multidisciplinary and governance meetings.
You will deliver departmental training and will provide clinical supervision and mentoring for junior pharmacists and medicines management technicians.
You will contribute to the quality training experience and support for trainee pharmacists, pharmacy students and trainees in the multi‑disciplinary team.
The post‑holder will carry out clinical pharmacy audit and practice research supported by the Lead pharmacist – medicine.
Main duties of the job
* Lead and coordinate the clinical pharmacy service in Acute Care areas by providing a highly specialised level of pharmaceutical care, including ward visits, patient consultations, prescription reviews, supply arrangements and liaison with other healthcare professionals.
* Practice as a highly specialised clinical pharmacist within local and national policies and standards to deliver medicines optimisation, conduct daily ward rounds and review individual patients’ treatment plans to determine the most appropriate medication.
* Work with medical, nursing and the site management team to facilitate patient discharges and maximise patient flow.

Qualifications, Experience and Skills
Essential
* Masters Degree in Pharmacy or equivalent
* Postgraduate Diploma in Clinical or Technical Pharmacy or equivalent demonstrable experience
* Working towards and will have completed Independent Prescribing qualification by commencement in post
* Registered as a Pharmacist with the General Pharmaceutical Council
Desirable
* MSc in Clinical or Technical Pharmacy
* Member of the Royal Pharmaceutical Society
Experience
* Extensive post‑registration experience with substantial hospital experience
* Extensive experience providing a pharmacy clinical service to patients in a range of Emergency and Acute Care Specialties
* Experience of providing and evaluating pharmacy education & training including mentorship
* Experience of implementing and/or reviewing policies, guidelines, PGDs or equivalent
Skills, Abilities and Knowledge
* Able to communicate effectively with a wide range of healthcare professionals and patients
* Able to provide expert advice on the use of medicines to treat patients within a specialist area
* Able to manage time effectively and meet tight deadlines
* Able to demonstrate an advanced level of reasoning and judgement
* Able to train and supervise staff
