Overview

As a Social Care Assistant working within the Home Independence Team, you will work out in the community to support residents recently discharged from hospital or those living with a disability who need help to live independently and establish a routine to manage day-to-day life. You will support individuals to regain confidence and reach their goals of living independently again. We are looking for dedicated individuals who can provide high-quality direct care and support to residents, focusing on personal wellbeing outcomes and individual wishes to achieve strength-based support. The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support designed to enhance client independence and meet their needs. We provide advice on specialist equipment options, monitor client progress and make necessary adjustments. No two days will be the same as you meet a variety of people and support their individual needs.


Responsibilities

* Providing people with information and advice in relation to care and support, and assistance in accessing care and support through various means.
* Working as part of a CQC registered service in the community, maintaining a high degree of autonomy and using initiative in delivering compliant care and support independently.
* Ensuring an agreed plan meets individual care and support outcomes, empowering the individual to access early intervention and preventative services.
* Informing senior colleagues and adhering to safeguarding procedures for any concerns related to the safety of vulnerable adults.
* Working in partnership with individuals, carers, service providers, voluntary sector and other professionals.
* Providing personal care for individuals, including dressing, toileting, washing and general housekeeping.
* Delivery of practical care and support to an agreed plan, using own initiative to problem solve and utilise remote management oversight where required.


Requirements / Qualifications

* Full UK driving licence is essential and access to a vehicle (these positions are community-based, not day centre based).
* Ability to work in a fast-paced environment and to build rapport with residents and families.
* Safeguarding awareness and commitment to promoting the welfare of children, young people and adults at risk.
* Proactive, compassionate, and empathetic with the ability to deliver care and support to an agreed plan.
* Right to work in the UK (role is not eligible for visa sponsorship).
* Bereavements regarding Enhanced DBS/Disclosure as applicable to the role (where required by law or policy).


Benefits and Additional Information

* Full-time: 37 hours per week, including alternate weekends and shifts between 7am – 10pm. Part-time: to be agreed with the hiring manager and will include every other weekend.
* Golden Hello: a one-off £1,000 payment upon successful completion of probation (pro rata for part-time hours).
* Salary may be accompanied by a car user allowance and HMRC mileage allowance (45p per mile).
