Come and join the expanding REACT (Rapid Emergency Assessment and Care Team) working in our Emergency Department front door areas, as an Occupational Therapist or Physiotherapist.
The successful candidate will provide very high quality assessments in order to establish robust and swift rehabilitation and discharge plans for the patients within the Front door footprint, predominantly to enable admission avoidance and return to the community with support from our expansive community services.
The successful candidates will work very closely with the Medical and Nursing colleagues, Discharge Co-ordinators, Hospital Social Workers, Speech and Language Therapists and Dieticians.
This is a very friendly, supportive and dynamic trust with regular integrated individual supervision and training, appraisal, CPD to support professional development.
Main duties of the job
1. To undertake assessments of patients with complex and diverse presentations taking into account their social needs outside of hospital.
2. To undertake advanced, comprehensive, and where applicable, standardised assessments in the clinical areas stated.
3. To participate within simple and complex rehabilitation and contribute to its function.
4. To use clinical reasoning skills and knowledge of evidence based practice to determine discharge and treatment plans using a wide range of treatment skills and knowledge of community service options.
5. To analyse and interpret clinical and non-clinical information to form an accurate plan for a wide range of conditions, with guidance from Therapists within the team and the MDT when appropriate.

Education, Qualifications and Training
Essential criteria
15. Current registration with Health Professions Council
16. Diploma /Degree in Physiotherapy
17. Evidence of CPD in the form of a detailed personal development portfolio
Desirable criteria
18. Field work/clinical educators course
19. Specific formal courses appropriate to post speciality
20. Membership of professional special interest group
Experience
Essential criteria
21. Experience of a junior rotational Physiotherapy post in the NHS
22. Experience of a broad rotational scheme including Acute medicine
23. Knowledge of areas of clinical governance including quality, audit and risk management
24. Experience of managing own caseload and prioritizing
25. Ability to work and interact effectively within an interdisciplinary team and contribute to decision making
26. Evidence of commitment to continuing professional development and reflective practice
27. Knowledge and application of evidence based practice and clinical reasoning
28. Establish and maintain CPD Portfolio
Desirable criteria
29. Experience of Community Physiotherapy
30. Attendance at Post graduate courses in related fields and demonstrates reflective learning
31. Knowledge of areas of influence of psychosocial and cultural factors on health/illness behaviours
32. Experience in supervision of junior staff
33. Participation in audit, research activities and development activities
34. Experience of undertaking Senior (Band 6) role
