Overview
You will be responsible for delivering quality care to a designated caseload of people in the place that they call home, following a care plan devised by the community nursing staff you will be working alongside. You will be asked to feedback concise information in a daily handover to ensure changing needs are met and risks escalated.

Responsibilities
* Be flexible and assist colleagues to cover service needs, including working across the wider integrated team.
* Attend to service duties which may require use of a car (access to a car is essential).
* Work in the community as a Health Care Assistant to support adults who live in Bedfordshire.
Requirements / Qualifications
* Dynamic, motivated and enthusiastic person with excellent communication, IT literate and problem-solving skills.
* Commitment to patient-centered care and compassionate service.
* Ability to work across a rotation of shifts, including weekends, evenings and Bank Holidays; flexibility to support service needs.
* Experience in community or NHS setting preferred, with willingness to learn and develop within the role.
Working hours
The Community Nursing Service operates between 08:00 and 21:00 hours, and the working pattern will include a variety of rotational shifts which will include weekend, evening and Bank Holiday working. Part-time posts would be considered.
