The Directorate of Primary Care, Older People’s Services and Nursing provides a range of homecare services designed to enable service users to remain at home and to live as independently as possible. Practical care services provision is achieved through empowering service users and carers to build on existing support networks. The focus of the Homecare Service is the provision of high quality care services in line with regulation and minimum required standards.Team Coordinators are an integral part of the team to which they are attached, they are expected to work closely with the Area Managers and other staff to ensure the delivery of a high quality service. Team Coordinators are office based and required to undertake the mainly electronic administrative tasks which underpin the delivery of good quality homecare so attention to detail is paramount.Please see employer website for full job details & person specification.
Responsibilities
* Process referrals for Care Services and arrange replacement service for service users when Homecare workers are unavailable due sickness, training, annual leave or any other type of leave.
* Make and receive telephone calls and emails and take appropriate action. This involves principally contact with Homecare workers, service users and other professionals with regard to service delivery arrangements.
* Accurate and clear notification to Homecare workers by telephone, electronically or in writing of the service they are to provide to service users. Responsible for preparing, checking and circulation of care rotas.
* Undertake work allocated from Area Managers and other senior staff.
Skills and Qualifications
* 4 GCSEs Grades A-C to include English Language (or equivalent or higher) plus one year’s relevant experience. Or Two years relevant experience.
* Competent in using Microsoft packages to include Word, Excel and Outlook.
