Job summary
Thepost holder will demonstrate courageous and critical thinking and is a caring,compassionate and committed experienced nurse who, acting within theirprofessional boundaries, will provide care for the presenting patient from initialhistory taking, clinical assessment, diagnosis, treatment and evaluation ofcare. They will demonstrate safe, competent clinical decision-making and expertcare, including assessment and diagnostic skills, for patients within thegeneral practice. The post holder will commit to demonstrating criticalthinking in the safe clinical decision-making process. They will communicateand work collaboratively with the general practice team to meet the needs ofpatients, supporting the delivery of policy and procedures, and providing nurseleadership as required.
Main duties of the job
1. Assess, diagnose, plan,implement and evaluate treatment/interventions and care for patients presentingwith an undifferentiated diagnosis
2. Clinically examine andassess patient needs from a physiological and psychological perspective, andplan clinical care accordingly
3. Assess, diagnosis, plan, implement andevaluate interventions/treatments for patients with complex needs
4. Proactively identify, diagnose and managetreatment plans for patients at risk of developing a long-term condition (asappropriate)
5. Diagnose and manage both acuteand chronic conditions, integrating both drug- and non-drug-based treatmentmethods into a management plan
6. Prescribe and review medication fortherapeutic effectiveness, appropriate to patient needs and in accordance withevidence-based practice and national and practice protocols, and within scopeof practice
7. Work with patients in order tosupport compliance with and adherence to prescribed treatments
8. Provide information andadvice on prescribed or over-the-counter medication on medication regimens,side-effects and interactions
9. Prioritise health problems and interveneappropriately to assist the patient in complex, urgent or emergency situations,including initiation of effective emergency care
