Main Duties of the Job
* Provide a high quality Physiotherapy service to patients referred to the Community Therapy team.
* Hold clinical responsibility for a designated caseload of patients.
* Undertake a high standard of Physiotherapy assessments of patients with diverse presentations using clinical reasoning skills to provide a diagnosis and deliver individualised treatment programmes.
* Provide advice and education to optimise health, wellbeing and to promote better self-management of a condition.
* Work as an autonomous practitioner within the multidisciplinary team.
* Ensure timely discharge of patients from the service and refer patients on to other health, social care and voluntary agencies as appropriate.
Qualifications
* Degree or equivalent in Physiotherapy.
Registration
* HCPC registration.
Driver
* Must be a car driver with access to a car and have a UK driving licence.
IT
* Knowledge of Systm One (desirable).
Experience
* Relevant clinical experience including student clinical placements.
Team Working
* Good understanding of the roles of other professionals.
Communication
* Able to communicate to a high standard in English both verbally and in writing.
