An exciting opportunity has arisen for an Occupational Therapist to join our Cardiac Rehabilitation Team in Carmarthenshire. The role supports the Health Boards strategy A Healthier Mid and West Wales, focusing on delivering effective, efficient, and sustainable services by shifting current hospital‑based care to community‑based settings.

Key Responsibilities
* Operate as an autonomous practitioner with a varied and often complex caseload, using specialist occupational therapy knowledge to plan, carry out and evaluate assessments and interventions.
* Carry out group education sessions on fatigue, stress, and anxiety management with individuals both virtually and in person.
* Collaborate as part of a multidisciplinary team to provide rehabilitation focused on the individual’s needs.
* Plan, organise and deliver therapeutic interventions with individuals or groups across hospitals, patients’ homes, educational establishments, and community & leisure facilities.
* Advise, educate and demonstrate specific techniques to individuals and their families/carers to support ongoing recovery management.
* Delegate clinical work to Support Workers and students, following relevant policies and guidelines.
* Supervise Occupational Therapy students on practice placements.
* Travel within Carmarthenshire and occasionally to wider Hywel Dda University Health Board areas; pool cars and mileage expenses are available.
Qualifications
* Current HCPC registration.
* Proactive, motivated, compassionate with demonstrable experience aligned to the values of Hywel Dda University Health Board.
* Excellent communication skills; the ability to speak Welsh is desirable, though English speakers are also invited to apply.
* Experience working autonomously and within multidisciplinary teams in a cardiac rehabilitation setting.
