Overview
We are currently looking for Paramedics with a minimum of 5+ years post registration to join a Primary Care Team in South West London (Croydon Area).
Role: Paramedic enhancing Primary Care, part-time.
Work schedule: 3 days per week (Tuesday, Wednesday and Thursday).
Location: South West London (Croydon Area).

Responsibilities
* Triaging and assessing patients in the community.
* Conducting home visits and working in clinics.
* Seeing patients with chronic and sometimes complex conditions and assisting with care plans.
* Making referrals while keeping comprehensive records using the required system.
* Working closely with GPs and other healthcare professionals to ensure appropriate care in the most suitable setting.
* Participation in a First Contact Practitioner (FCP) pathway, fully supported by NorMed UK.
* Acute and chronic disease management, home visiting service, care home ward rounds, on-the-day clinics for acute conditions, advanced care planning, long-term condition reviews, ECGs, BP checks, phlebotomy, and health checks for learning disability or dementia as applicable.
* Visiting house-bound or vulnerable patients as part of primary care delivery.
Requirements
Essential:
* HCPC registered Paramedic, 5 years post-qualification.
* Experience dealing with non-acute patients in the community.
* Willingness to complete essential development and portfolio work.
Desirable:
* Urgent and Primary Care HEE level 7 education.
* Experience working in a Primary Care setting.

Compensation and Location
Pay: £51,000.00-£57,000.00 per year (pro-rated for part-time).
